I'm going to suggest the HP NC365T 4-port (Intel chipset) NIC.

 

I've seen them on eBay for $50-100.  Low profile is an option.

You'll get the best driver support and throughput having things all on one card.  I'm contemplating one for my ML310e at some point, but between the onboard 2 ports and an Intel dual-port PCIe, I'll probably hold off for a little bit.
